Every patient registered with a GP practice has the legal right to access their medical records for free.
Your records may include appointment history, test results, referral or hospital discharge letters, vaccination or childhood immunisations.
You can also nominate someone you trust to have access to your records if you need to.
Records are available digitally through our online service, SystmOne and the NHS App.
